The job is home-based nursing. Older clients on in-home aged care, plus NDIS participants. You keep them healthy and independent, and you fit the care to each person, not the other way around. You work on your own a lot, so you need to back your clinical calls.

Key Responsibilities
- Nurse aged care clients and NDIS participants in their homes
- Assess clients clinically, then build and review care plans
- Handle medication support, watch for health changes and keep clinical records accurate
- Work with clients, families, carers and other health professionals, putting the client's choice, dignity and independence first
Requirements & Eligibility
- Current AHPRA registration as a Registered Nurse
- Relevant experience in community nursing, aged care or disability services
- Strong clinical assessment, communication and organisational skills
- Confident working independently in a community setting, with compassion and commitment to safe, person-centred care
- Willingness to move to the Northern Territory
