Care Partner / Case Manager
Visa sponsorship
Community Services · Sydney, NSW, Australia · Full-Time · $80,000 - $95,000/yr · Visa: Employer sponsored
No fluff here. A Sydney aged care provider wants a Care Partner / Case Manager who knows how to run a caseload. The organisation provides person-centred care to older Australians at home, funded through the Support at Home program, and has a small, close team.
You deal with clients directly. You lead the care coordination, you manage each person's support plan and you make decisions without waiting for sign-off, because the structure is flat. The job is done well when clients get what they need, when they need it. Visa sponsorship is also available for the right candidates.
